Legacy uses the existing round-robin board. AI Assist queues patients, respects spacing, and tries to balance patient load.
The first two patients of a doctor's first shift hour still go immediately. Outside that start-hour window, the AI bucket will not give any doctor a second patient in the hour before the other active doctors have at least one.
Fairness buffer waits briefly for other doctors who are about to become eligible. Load-first mode compares only the doctors currently in the active rotation and uses legacy board order only as a tiebreaker when the fairness gap is small.
The load-first pairing toggle only affects AI Assist load-first routing. Local keeps the current deterministic matcher; Zen lets MiniMax choose the doctor-patient pair and falls back locally if Zen is unavailable.
Each physician can choose a spacing preference. Waits under the default are treated as "complex-patients only" to prevent gaming easy patients.
